SHIP and Healthcare

SHIP 4.3 (September 2017 – October 2018) has two healthcare components:

  • Worksite Wellness
  • Breastfeeding Coalition

Worksite Wellness

This component involves public health partnering with healthcare settings to encourage participation in the PartnerSHIP 4 Health 2018 Worksite Wellness Collaborative scheduled to begin in January of 2018.

Breastfeeding Coalition

This component is public health partnering with healthcare to support Partnership4Breastfeeding, the local breastfeeding coalition. Current coalition members are staff members representing Lake Region Healthcare, Perham Health, and Otter Tail County Public Health.

Community Wellness Grant and Healthcare

PartnerSHIP 4 Health, through the Community Wellness Grant (CWG), seeks to impact the local populations at-risk for diabetes and high blood pressure (low-income and/or ethnic minorities).

The Community Wellness Grant (CWG) has three components related to healthcare:

  • Health System Intervention
  • Community Clinical Linkages
  • Lifestyle Change

With its inaugural Hispanic Diabetes Prevention Program in 2015, the Pelican Rapids Hispanic community gained a new tool to combat diabetes. Participants are modeling healthy behaviors and getting the word out to the community on how they improved their health. By partnering with community groups and faith communities, PartnerSHIP 4 Health hopes to reach more and more Hispanics each year. Health System Interventions

This component involves public health partnering with healthcare settings to address prediabetes and hypertension among their patient population by putting scientific-based guidelines into practice. Health care partners are empowered to integrate the steps of “Ask, Advise, Assess Readiness, Assist, and Arrange Follow-up” related to prediabetes and hypertension into clinic and practice flow.

In addition, because a “culture of wellness” enhances clinician ability to have wellness discussions with their patients, PartnerSHIP 4 Health works with healthcare partners to advance worksite wellness in their respective settings by encouraging participation in the PartnerSHIP 4 Health Worksite Wellness Collaborative.

Community Clinical Linkages

This component is public health partnering with organizations that employ Community Paramedics, Community Health Workers, and Community Pharmacists to engage in hypertension management and to develop a bi-directional referral system related to the I CAN Prevent Diabetes Program.

Lifestyle Change

This component is public health partnering with worksites, community organizations, and healthcare settings to offer diabetes prevention to our priority populations (low-income and/or ethnic minorities). Together we build support for the Diabetes Prevention Program (DPP), known as I CAN Prevent Diabetes Program in Minnesota, via development of a Local Diabetes Prevention Network (LDPN), training of DPP Lifestyle Coaches, technical assistance for DPP providers, and tailored communications and incentives.

MN Diabetes Prevention Call Line: Call 1-888-643-2584 to speak with a Diabetes Prevention Navigator about local DPP classes as well as non-DPP options related to diabetes prevention.
