About Us

PartnerSHIP 4 Health (PS4H)

We are a collaboration of community partners in Becker, Clay, Otter Tail, and Wilkin counties working to reduce costs associated with chronic disease and improve the health of our residents through preventive strategies that support sustainable changes in our schools, communities, workplaces, and health care.

PS4H received its start through the Statewide Health Improvement Program (SHIP) which is the prevention component of Minnesota’s 2008 Health Care Reform. It is designed to help Minnesotans live longer, better, healthier lives by preventing the risk factors that lead to chronic disease. PS4H works with Schools on:

Farm to School, Safe Routes to School, Physical Activity, Tobacco Free Campuses

PS4H works with Communities on:

Increasing access to local foods, increased non-motorized transportation, healthier eating and physical activity in childcare, tobacco free parks, smoke free multi-unit housing

PS4H works with Worksites on:

Comprehensive workplace wellness policies & practices to improve nutrition, increase physical activity and promote smoking cessation. These strategies improve employee health while reducing absenteeism and the health care costs incurred by employers.

PS4H works with Health Care on:

Reduce obesity and tobacco use among their patient population by putting scientific-based guidelines into practice. Health care partners are empowered to integrate the steps of “Screen, Counsel, Refer, and Follow-up” related to body mass index (BMI), nutrition, physical activity, and tobacco use/exposure into clinic and practice flow.

PS4H accomplishes its goals by:

Implementing the U.S. Centers for Disease Control and Prevention’s best practice strategies to save lives and state resources.

Implementing policy, system, and environmental changes which have proven to be the most effective and efficient tools in public health. They create opportunities for sustaining influence on the health behavior of the public.
